Evaluation records are stored within my secure clinical record system and maintained as part of your file. They are not automatically shared with hospitals, primary care providers, schools, or government agencies. Your evaluation does not enter a larger medical network unless you choose to provide the report to another party. I maintain records in accordance with professional record-keeping and retention standards.
No one automatically receives your diagnosis outside this practice. I do not send reports to employers, schools, family members, or other providers unless you request it.
Most sharing of your report requires your written authorization. This includes sending the report to an employer, physician, school, disability office, testing agency, or any third party. You may choose to share the full report, a summary letter, or nothing at all. You can also revoke a release at any time for future disclosures.
I am required to comply with legal orders such as court subpoenas and mandatory reporting laws. These situations are uncommon and governed by state and federal regulations rather than clinician discretion. Outside of these legally defined circumstances, information is released only with your authorization.
You have options in how documentation is created and used:
• You may pursue a full diagnostic evaluation with written report
• You may request a consultation without diagnostic documentation
• You may choose whether to use insurance or pay privately
• You decide if, when, and to whom the report is released
• You may request targeted letters rather than sharing the full report
Many adults choose an approach based on how they plan to use the information (personal understanding, accommodations, medical coordination, or documentation). We can discuss these options before scheduling so you can select the level of documentation that fits your needs.
